Enrollment Form Reminder

CEDI maintains the enrollment forms for electronic transactions to Medicare for all four DME MAC Jurisdictions. To enroll in one or multiple Jurisdictions, you will only need to submit one set of forms and CEDI will process them for all four DME MAC Jurisdictions. The enrollment forms are located on the CEDI Web site at www.ngscedi.com. These forms are completed and submitted electronically online, then printed, signed, dated, and faxed to the CEDI Enrollment Department to be processed. CEDI has Instructional Guides listed with each enrollment form to assist in completing the forms and when each form would be required.
