This inquiry tool gives you an update on your application instantly and can be used to check the status of the following common electronic data interchange (CEDI) enrollment forms:
- CEDI Enrollment Form
- CEDI Submitter Action Request Form
- CEDI Supplier Authorization Form
How to Search
Step 1: Choose the Search Method
There are two ways to search for your forms.
- With Request ID: You can search by the Request ID (RID) number located on the form; this was also included in your confirmation e-mail
- Without Request ID: You can search by the National Provider Identifier (NPI), Provider Transaction Access Number (PTAN), and request date; the request date is the date the form was submitted electronically
- Note: All three are required for this search method.

Step 2: Select the Form Type
You will need to select the type of form you submitted.
There are three forms you can check status for:
- CEDI Enrollment Agreement
- CEDI Submitter Action Request
- CEDI Supplier Authorization
Use the drop-down arrow to the right of the box to select the Form Type.

- If you chose With Request ID, proceed to Step 3
- If you chose Without Request ID, proceed to Step 4
Step 3: Search With Request ID

Step 4: Search Without Request ID
- Enter the request date; this is the date of the electronic submission of the form
- Enter the PTAN located on the form
- Enter the NPI located on the form
- Note: All three fields are required for this search.

How to Read the Search Results

- The status date is the date when the status of the form was last changed
- The original faxed received is the date the CEDI enrollment department received the faxed copy of the request forms
- The current status indicates at what point the form is in process: Pending, Approved, Completed or Rejected
- If the form is rejected, the rejection reason will be indicated in the rejection reason field
- Notes will be used to indicate additional information about the rejection
